Midface Ratio: What It Measures and How to Calculate Yours

Midface ratio compares the height of the middle section of your face to its width. A lower ratio means a compact midface. A higher ratio means a long one. It affects how the eye area and the lower face read relative to each other, and it is set skeletally.

Midface height is measured from the pupil line, meaning the horizontal line through the centre of both pupils, down to the upper lip.
Midface width is the bizygomatic width, the distance across the widest point of the cheekbones.
Height divided by width gives the ratio.
Most adults fall roughly between 0.9 and 1.1, though reported figures vary with the exact landmarks a given source uses. Below about 0.95 is generally described as compact. Above about 1.05 as long.
Note that other definitions exist. Some sources measure midface height from the nasion, the bridge of the nose, or to the subnasale rather than the upper lip. These produce different numbers for the same face, which is the main reason midface ratio figures disagree across tools.
What a compact or long midface changes
This is a proportion, not a quality, and it interacts with the rest of the face rather than standing alone.
A compact midface brings the eye region and the mouth closer together relative to face width. The eye area tends to read as more prominent, because there is less vertical space around it. The face tends to read as broader.
A long midface creates more vertical distance between the eyes and the mouth. It can make the eye area read as smaller relative to the face and shifts visual weight toward the centre.
The reason this measurement gets attention in facial aesthetics discussion is that it interacts strongly with eye area perception. Two people with identical eye measurements can have noticeably different-looking eye regions because of the vertical space around them.

Why different tools give different numbers
Landmark definition, which is the main issue here. As above, sources disagree on where the midface starts and ends. Pupil line against nasion, upper lip against subnasale. Each combination gives a different number, and none is wrong so much as differently defined.
Landmark sensitivity. Midface ratio depends on pupil centres and a lip landmark, and small placement errors in either move the ratio noticeably because the measured distance is short relative to the precision available. A two pixel error matters more here than it does on face length.
Head pitch. Chin up or down compresses or extends midface height while leaving bizygomatic width unchanged, which changes the ratio directly. This is the most common source of variation between two photos of the same person.
Camera distance. Close-range perspective distortion widens the centre of the face, inflating bizygomatic width and lowering the ratio.

Because this metric is landmark-sensitive, confidence tends to run lower here than on measurements with longer baselines. That is the measurement being honest, not the tool failing.

What changes it
In adults, essentially nothing without surgery. Midface height is determined by maxillary vertical dimension and orbital position. Bizygomatic width is determined by zygomatic bone structure. Both are skeletal and stable once growth is complete.
Surgical options exist and are significant. Le Fort osteotomies reposition the maxilla and are performed primarily for functional bite correction with proportional effects. Zygomatic implants or reduction alter width. These are maxillofacial procedures with substantial recovery, and they belong in a conversation with a surgeon, not with a website.
Things that change appearance without changing the ratio: midface volume, which declines with age and can be supplemented medically, alters how the midface reads without moving the underlying measurement. Body fat affects apparent cheek width slightly.

How to read your result
Midface ratio is one of the less immediately actionable measurements on this site. It is not something you change, and it does not map to a styling decision the way face shape does.
Where it earns its place is in explanation. If you have wondered why your eye area reads a certain way, or why your face looks broader or longer than the individual features suggest, midface ratio is frequently the reason. It contextualises other measurements rather than standing on its own.
